SMLE Exam Day Rules 2026: The Official SCFHS Protocol Every Candidate Must Know

Written by the SMLEREVISE Clinical Team  ·  Updated June 2026  ·  15 min read

⚡ TL;DR — Exam Day Essentials

  • Arrive by 7:30 a.m. at the latest. Being more than 30 minutes late is a "No Show" and counts as an attempt.
  • Bring a valid, unexpired passport, Saudi ID, or Resident ID — the name must exactly match your scheduling permit.
  • The exam is 3 blocks of 100 questions (120 min each), with 45 minutes of pooled breaks between them.
  • Security includes fingerprint and photo ID scans every time you enter or leave the testing room.
  • Writing materials are provided — you cannot write on your hands, tissues, or any unauthorised material.
  • Phones and recording devices are prohibited even if turned off. Possession can trigger an irregular behaviour finding.

1. Why the Official SCFHS Guide Matters

Most SMLE candidates prepare extensively for the clinical content of the exam but under-prepare for the logistical and regulatory realities of exam day. A single violation of Prometric or SCFHS protocol — arriving 35 minutes late, bringing a phone into the testing room, or writing on your hand instead of the provided tablet — can invalidate your attempt, cost you the full exam fee, and consume one of your limited annual attempts.

This guide is built directly from the SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13), the official document that governs every aspect of the SMLE examination. Unlike forum posts, Telegram group anecdotes, or third-party blog summaries, the rules here are the actual rules enforced by the SCFHS and Prometric. When in doubt, the official guide prevails.

"Candidates must acknowledge they have read and understood the [SMLE Applicant] Guide before submitting an application." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

2. Before You Leave Home: The Final Checklist

Use this checklist the night before your exam. Print it or save it to your phone (then leave the phone in your car or a locker).

  • Valid, unexpired ID — passport, Saudi national ID, or Saudi resident ID (Iqama). The name must match your scheduling permit exactly.
  • Scheduling permit — printed or electronic copy showing your exam date, time, and centre.
  • No phone — not in your pocket, not in your bag, not "just for the Uber." Leave it at home or in your car.
  • No smartwatch or fitness tracker — these are treated as electronic devices.
  • Comfortable layers — Prometric centres are often air-conditioned. A light jacket or sweater is advisable.
  • Light snacks for break — you cannot eat in the testing room, but you can during breaks. Protein bars, bananas, and water are ideal.
  • Personal medication — in original packaging, with prescription if applicable. Declare this at check-in.
No ID + No permit = No admission. If you forget your ID or your permit name does not match, you will be turned away and it will count as a No Show. This is non-negotiable at every Prometric centre worldwide.

3. Arrival, Check-In, and the 30-Minute Rule

Test centres open at 7:30 a.m. You should aim to arrive at least 45 minutes before your scheduled start time to complete check-in without rushing. Check-in involves:

  • Presenting your ID and scheduling permit
  • Biometric capture (digital photograph and fingerprint scan)
  • A physical screening (pockets, sleeves, glasses check)
  • Storing all personal belongings in a locker
  • Signing an examinee agreement
"Arriving more than 30 minutes late is considered a 'No Show' and counts as an attempt (unless an acceptable reason with documentation is approved by the committee)." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

The No Show rule is absolute. Traffic, flight delays, alarm failure, or a flat tyre are not automatic exemptions. The SCFHS states that an "acceptable reason with documentation" may be approved by the committee — but this is discretionary, not guaranteed, and the appeals process takes weeks. Plan your travel conservatively. If your centre is 30 minutes away, leave 90 minutes early. If you are flying in, arrive the day before and stay within walking distance.

Real-world tip: Candidates who test in Riyadh or Jeddah during rush hour frequently miss the 30-minute window. Book an early exam slot (8:00 a.m.) and stay within 10 minutes of the centre the night before.

4. Security Protocol: What to Expect

The SCFHS and Prometric take exam security extremely seriously. The SMLE is a high-stakes licensing exam, and every candidate is subject to the same continuous monitoring protocol:

Continuous Monitoring

The testing room is under continuous video and audio recording for the entire duration of the exam. Physical walk-throughs by test centre administrators (TCAs) occur regularly. You are observed through observation windows and on CCTV at all times.

Entry and Exit Scans

Every time you enter or leave the testing room — including after each break — you must:

  • Present your ID
  • Have your fingerprint and/or photo scanned
  • Sign in and out on the log sheet
  • Submit to a brief physical screening
"Each time entering/leaving the testing room, including after breaks, [candidates undergo] fingerprint and photo ID scan. Must sign in/out." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

This process can take 10–15 minutes based on testing centre capacity. Factor this into your break planning. If you take a 20-minute break and the sign-out/sign-in process takes 12 minutes, you have effectively had 8 minutes of actual rest.

Writing Materials

Prometric provides laminated writing surfaces or e-tablets, erasers, and markers. You cannot bring your own paper, pens, or whiteboards. You also cannot write on your hands, arms, tissues, or any other unauthorised material. Doing so constitutes irregular behaviour and may be reported.

5. The Real Exam Structure: 3 Blocks, Not 2

One of the most persistent misconceptions about the SMLE is that it consists of "two sessions of 150 questions." The official SCFHS guide is explicit: the exam is delivered in three blocks of 100 questions each, with 120 minutes per block.

Component Official SCFHS Specification
Total questions 300 MCQs + up to 20 pilot (unscored) items
Blocks 3 blocks of 100 questions
Time per block 120 minutes (2 hours)
Breaks 2 scheduled breaks with 45 minutes total (pooled)
Options per question 4 (choose one best answer)
"The SMLE is a 300-MCQ computer-based examination (may include up to 20 pilot/unscored questions). Six hours total, divided into three sections of 100 questions each (120 minutes per section)." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

The pilot items are indistinguishable from scored items. You cannot identify them, and they do not affect your result. Treat every question as if it counts.

Strategic implication: Because there are three blocks (not two), you experience two mental "resets" rather than one. Your stamina must carry you through three 2-hour sprints, not two 3-hour marathons. Train accordingly — do 100-question mock blocks, not just 150-question sessions.

🎯 Train With the Real 3-Block Structure

The SMLEREVISE Scaled Grand Mock mirrors the official SCFHS exam structure: 3 blocks of 100 questions, 120 minutes each, pooled breaks, and Prometric-identical interface. Train under real conditions so exam day feels familiar.

6. Break Rules: The 45-Minute Pool

The SMLE provides 45 minutes of total break time, pooled across two scheduled breaks between blocks. You decide how to divide it. You might take 20 minutes after Block 1 and 25 minutes after Block 2 — or 15 and 30, or any combination totalling 45 minutes.

The critical rule: if you exceed the 45-minute pool, the excess time is deducted from your next block. The computer tracks your break time automatically. The next block starts automatically when your break time is exhausted. There is no warning bell — the timer simply resumes.

"Two scheduled breaks with a pooled total of 45 minutes (candidate can divide between the two breaks). If break time exceeds the 45-minute pool, the excess is deducted from the next block's testing time. The next block starts automatically when break time is exhausted." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

Additionally, candidates should arrive at the testing room 10–15 minutes before the next block starts to allow time for the sign-in security check, which can take ~15 minutes depending on testing capacity.

Break strategy: Take both breaks. Use the restroom. Eat a small snack. Drink water. Close your eyes for 2 minutes. Do not use break time to review notes (you cannot access them anyway). The candidates who perform best in Block 3 are invariably those who took structured breaks, not those who powered through.

7. Prohibited Items and Irregular Behaviour

The SCFHS divides prohibitions into three phases: before the exam, during the exam, and after the exam. Violations in any phase can result in score cancellation, reporting to regulatory bodies, and legal prosecution.

Before the Exam

  • No unauthorised access to exam materials
  • No false information on applications
  • No communication about specific test items with examiners or item developers

During the Exam

  • No proxy testing or taking the exam when ineligible
  • No unauthorised assistance from any source
  • No notes on anything other than provided materials (no writing on hands, tissues, etc.)
  • No failure to follow TCA instructions
  • No harassment of staff
  • No phones, cameras, or recording equipment — even if turned off
  • No communication about test items with other examinees

After the Exam

  • No altering or misrepresenting scores
  • No reproducing or disseminating copyrighted exam materials — including via memorisation or internet posting
  • No communication about specific test items with others
  • No failure to cooperate in investigations
"Exam content is copyrighted and confidential. No reproduction by any means (including memorization/reconstruction). No posting of content/answers on the internet." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)
Irregular behaviour findings are career-altering. The SCFHS can share findings with other health regulatory bodies, which may affect your ability to obtain licences in other jurisdictions. A finding of irregular behaviour is far more damaging than a failed exam score.

8. Leaving Early and Finishing the Exam

You cannot leave the examination hall before 30 minutes have elapsed from the start of the exam. This rule applies even if you feel unwell or have an emergency. If you must leave, you will be accompanied by an invigilator and an incident report will be filed.

Within a block, you can leave your seat to use the restroom — but this triggers an unauthorised break screen and may be reported by the TCA if it is repeated or lengthy. The block timer continues to run while you are away. Do not leave during a block unless it is a genuine emergency.

"Cannot leave examination hall before 30 minutes have elapsed. Must be accompanied by invigilator if leaving and returning. Unauthorized break screen appears after defined inactivity." — SCFHS SMLE Applicant Guide 2022 (Guide_0-1-13)

Once you finish the final block, you can end the exam. There is no penalty for finishing early — but there is no benefit either. Use all available time to review flagged questions. Many candidates change 2–3 answers on review, and those changes often move them from below 560 to above it.

9. What Happens After the Exam

After you submit your final block, you will complete a brief post-exam survey. Then you will collect your belongings from the locker and leave the centre. You cannot cancel or reschedule after starting the exam.

Results are not instant. The SCFHS conducts psychometric analysis during the window closing period. Results are announced 2–6 weeks after the end of the testing window. You will receive two documents: a Statement of Results (scaled score + pass/fail) and a Performance Feedback Report (domain-level comparison to other test-takers).

10. Frequently Asked Questions

Can I wear a hijab or head covering?

Yes. Religious head coverings are permitted but may be subject to inspection by TCAs to confirm no unauthorised items are concealed. Inform the TCA at check-in if you wear a head covering for religious reasons.

What if I feel sick during the exam?

Inform the TCA immediately. Minor discomfort (headache, nausea) can sometimes be managed with over-the-counter medication from the centre's first-aid kit. Serious illness may require you to stop the exam, but this is reported as an incomplete attempt and you will need to reschedule and pay again. If you are unwell on exam day, consider rescheduling before the exam begins.

Can I bring a water bottle?

No. Water and snacks must remain in your locker and can only be consumed during breaks in the designated break area. Some centres provide water fountains — ask your TCA.

What happens if the computer crashes?

Prometric's systems auto-save your responses every few seconds. If a technical failure occurs, the TCA will restart your session and you will resume exactly where you left off. The lost time is typically added back to your block timer. Report any technical issue to the TCA immediately — do not attempt to fix it yourself.

Can I request extra break time for medical reasons?

Yes, but this must be arranged in advance through SCFHS and Prometric's accommodation services. Medical accommodation requests require documentation from a licensed physician and should be submitted at least 4–6 weeks before your exam date.

Is there a dress code?

No formal dress code, but dress comfortably and in layers. Avoid clothing with large pockets or bulky items that slow down security screening. Shoes must be worn at all times.
